Patient Responsibilities

Your complying with the following list of Patient Responsibilities is necessary to assure you of the highest quality care.

You are responsible for the following:

  • To tell the doctor or provider everything you can about your condition, including all symptoms, the medicines you are taking, and your past medical history.
  • To ask questions about your care when you do not fully understand information or instructions.  Understanding your health problem(s) is important to the success of your treatment.
  • To follow the treatment plan as outlined by your provider.  Keep appointments and notify us when you are unable to do so.
  • To let the doctor or provider know about changes in your symptoms, medications or general conditions.
  • To be considerate of the rights of other patients, clinic personnel and property.
  • To follow the Clinic's safety rules and posted signs.
  • To provide Mile Bluff Clinic, LLP with accurate and timely information concerning your sources of payment and ability to meet your financial obligations for healthcare services received.
